Community leaders in Milford, Massachusetts identified a lack of access to health care services as a problem for the city. The Greater Milford Health Access Coalition (GMHAC) has been formed to develop and implement a plan to improve access to health care to underserved persons in its catchment area. GMHAC's plan was to work collaboratively to open a satellite community health center. JSI served as the project evaluator. The Health Foundation of Central Massachusetts awarded the Milford Regional Medical Center (MRMC) a second-year pilot year grant through its Synergy Initiative. Synergy Initiative projects typically last five years, and JSI evaluated activities throughout the program period.
