Harbor Homes is a non-profit community-benefit organization that provides low-income, homeless, and disabled New Hampshire community members with affordable housing, primary and behavioral health care, employment and job training, and supportive services. JSI will provide evaluation and quality improvement technical assistance and services to Harbor Homes, Inc. to support a three-year project funded by SAMHSA. Harbor Homes, Inc. is using the funding from SAMHSA to develop and implement an expanded array of integrated services for substance use, co-occurring disorders, permanent housing and other critical services for homeless veterans and other chronically homeless individuals and families.
In collaboration with Harbor Homes’ internal QI specialist, JSI will identify existing performance assessment and evaluation instruments and methodology or develop these as needed; provide technical assistance in data collection and management; periodically assess project progress as identified within the evaluation plan and grant reporting requirements; analyze and report process and outcome data; and produce performance assessment and evaluation updates, annual and/or biannual reports, and a final project report for a variety of audiences, including project steering committee, Harbor Homes administrators, community stakeholders, and SAMHSA.
