The Portland Community Health Center (PCHC) was established in 2009 to provide comprehensive, culturally competent, quality primary health care services to the medically underserved low-income, homeless, and immigrant populations in the city of Portland, Maine.

JSI was contracted by PCHC to help complete its Service Area Competition Grant application (HRSA, Section 330) in order to provide continuation funding for the health center. The grant requires detailed descriptions of the population needs within the service area, the CHC extended services including clinical performance measures and QI efforts, and the management, finance, and governance structures of the organization.

JSI researched the new grant application requirements for fiscal year 2013, framed the grant format, accessed the necessary information from multiple sources including PCHC, supported the writing of the narrative and assisted PCHC staff with the electronic submission process.
