Colorado is a leader in implementing health care reform for its Medicaid population. In 2011, Colorado's Department of Health Care Policy and Financing set up Regional Care Collaborative Organizations (RCCOs) across the state to act as a conduit for Medicaid payment reform. The RCCOs provide a client-centered approach to managed care that is focused on delivering efficient and coordinated care that improves the overall health of clients. This model of care differs from capitated managed care by investing directly in community infrastructure to support care teams and care coordination and creates aligned incentives to measurably improve client health and reduce avoidable health care costs.

Integrated Community Health Partners (ICHP) is the RCCO for southern Colorado. It is composed of the Community Health Centers and Mental Health Centers that serve southern Colorado. Upon nearing the completion of its first year as the RCCO for southern Colorado and looking forward to its second year, ICHP decided to conduct a planning process to provide the organization with priorities to pursue over the next year as well as set direction for the organization in the long term. JSI was contracted to support the organization in its planning efforts.

As part of this project, JSI provided strategic planning facilitation and research support to ICHP. JSI conducted an environmental scan using electronic survey methods and compiled the results for input into the strategic planning process. JSI also worked with ICHP's Board to identify strengths, weaknesses, opportunities and threats and distill the information from the environmental scan into focus areas for ICHP. Based on the Board's deliberations, JSI drafted a strategic plan for ICHP to implement in order to remain a critical component of the health care system in southern Colorado.
