The Routine Health Information System (RHIS) Rapid Assessment Tool—consisting of this Data Entry Module and also the Analysis and Dashboards Module—is designed to identify gaps in the RHIS during the health-sector planning process, so that deficiencies can be addressed during strategic planning. The tool is supplemented by an implementation guide.

The RHIS Rapid Assessment Tool focuses on routine data systems. Both tools build on the method to strengthen the M&E platform that is presented in WHO's "Monitoring, Evaluation and Review of National Health Strategies: A Country-Led Platform for Information and Accountability."

The RHIS Rapid Assessment Tool was developed by MEASURE Evaluation (in collaboration with WHO's Information, Evidence and Research Department. Substantial input was made by other WHO departments and programs, health ministries, regional experts, and global and in-country partners. JSI/MEASURE Evaluation. 2018.
