Infographic: HIV Prevention Continuum – Documenting Referrals for Clients at High-Risk for HIV Infection

This infographic reflects a range of social determinants and services that contribute to a client’s ability to successfully navigate HIV prevention activities. Community Based Organizations must ensure they have a process in place to assess the needs of clients who are at high risk for HIV infection, and then either provide appropriate services or refer clients to those services.
