SUMMARY

In February 2020, the Connecticut Department of Health contracted with JSI Research & Training Institute, Inc., to build the capacity of three Federally Qualified Health Centers and one Visiting Nurse Association throughout the state to implement the Center for Disease Control and Prevention’s (CDC) DP18-1815 diabetes and heart disease prevention and management grant strategies. The DP18-1815 grant aspires to implement and expand evidence-based strategies that address heart disease (specifically hypertension and hypercholesterolemia) and Type 2 diabetes by integrating and enhancing population health activities.

JSI is currently in the process of assessing the healthcare organizations’ EMR usage and clinical operations and workflows in order to identify recommendations for quality improvement, as well as provide technical assistance to the organizations to implement DP18-1815 strategies based on needs and assets identified in the assessment.
