Advancing Team Based Care through Technical Assistance

In February 2020, the Colorado Health Foundation launched a funding opportunity for safety-net clinics in Colorado. They partnered with JSI, Nonprofit Finance Fund (NFF), and Think Equity to assist clinics in integrating a TBC (Team-Based Care) model ensuring its financial, equity, and operational sustainability especially in light of the COVID pandemic. We are working with 16 clinics across the state providing coaching, peer-learning events and subject matter expertise.

The Advancing Team-Based Care initiative brings together experts from various fields to support clinics in achieving the quintuple aim: better care, happier patients and staff, sustained costs and equity. It emphasizes collaboration among healthcare organizations to enhance patient outcomes and resource utilization; and enhances access to healthcare services in underserved areas by leveraging the skills of diverse team members. It is also a great test case for how collaboration among technical assistance (TA) providers contributes towards better more sustained health outcomes.
