Health care providers that serve as the primary health care safety net in metro-Atlanta--including Grady Hospital and four Federally Qualified Health Centers (FQHCs)--are working collaboratively to strengthen and expand health services for low income and medically underserved residents. With the support of the local philanthropic community, the collaboration is aiming to reduce unnecessary emergency room use by linking patients to community-based primary care medical homes, to expand primary care services, and to better coordinate care among Grady and the FQHCs. JSI provided consulting services to develop a business plan for a new walk-in center within Grady Hospital, assisted in developing and analyzing data on primary care need and capacity and assessed options for expanding FQHC services.
