Transition care is gaining recognition as a key ingredient to achieving Triple AIM - better care and better outcomes with lower costs. Home care agencies are skilled at providing effective transition care (from hospital to home and from treatment to hospice care) and are fast becoming strategic partners within accountable care organizations and other integrated delivery systems.

JSI was engaged by Community VNA, a full service home health agency, to lead the development of strategic options to effectively respond to these market changes triggered by the passage of the Affordable Care Act as well as health care reform initiatives within the Commonwealth of Massachusetts, including patient centered medical homes initiatives and payment reform through global payment. JSI worked closely with the Community VNA Strategic Planning Committee to set a strategic agenda, evaluate findings from internal and external analyses, and formulate strategy options. JSI continues to work with Community VNA to move forward on implementation and refining strategies in collaboration with integrated delivery systems as strategic partners.
