The HIV/AIDS epidemic continues to affect many people in Santa Clara County, California, and beyond. JSI developed the 2012-2014 Comprehensive HIV Prevention & Care Plan and a Quality Management (QM) Plan for the Santa Clara County Public Health Department, part of the Santa Clara County/San Jose Transitional Grant Area (TGA). This scope of work included pre-planning; data gathering and analysis; plan preparation and approval; and an implementation plan. As part of this work JSI also conducted a needs assessment. Throughout the project, JSI worked collaboratively with the Santa Clara County HIV Planning Council for Prevention and Care. The Comprehensive HIV Prevention & Care Plan and QM Plan are living tools that will assist the Santa Clara Public Health Department and the HIV Planning Council to guide programming over the next several years.
