The National Association of Community Health Centers (NACHC) is interested in capturing the details of alternate payment methodologies (APM) payment reform models--a type of alternative payment model only available to FQHCs--as they are pursued by primary care associations (PCAs) and health centers in various states.

To date Oregon and California have developed APM payment reform models. Primary care associations in other states are interested in understanding the details of current models to inform their own payment reform efforts.

JSI will develop a summary template that can serve as a standardized format for describing specific state health center APM models and, where appropriate, why each model was designed as it was. The template will be designed so that it could be completed by different people in the future. Each section will include questions or prompts regarding the information to be included.

JSI will use the template to describe California’s APM model. Completion of the template will be based on discussions with CA PCA lead policy staff involved in the model design, review of documents describing the APM model, and conversations with JSI staff involved in the development of the model.
