Health reform calls for a shift from individually-focused medical care to a population approach to health. This represents a new horizon for many in the medical sector, presenting a need for tools that support the process of improving population health. This is not new for the public health arena, however, and existing tools may be adopted to address this need.

JSI collaborated with The Dartmouth Institute for Health Policy and Clinical Practice (TDI) to identify existing public health quality improvement models and frameworks, and related tools, to provide a resource for health professionals who are faced with a new challenge of improving population health.

As part of the Geisel School of Medicine, TDI focuses on education and research, including evaluating health care, with a particular focus on the health care system.
