In order to reduce unnecessary emergency department admissions, JSI assisted with an evaluation of Milford Regional Medical Center Emergency Department data to support redirect of (non-emergent) ED patients to the soon-to-open community health center.

As part of this process, JSI conducted an evaluation of baseline ED data for patients for last quarter (October – December) of 2013 with an analytic focus on patients with public insurance or no insurance, patients without a primary care provider, and high utilizers. A similar report was produced for ED data for patients for the first quarter (January – March) of 2014, after the health center opened.

In addition, JSI provided technical assistance (TA) on the development of a referral system to track patients referred to Edward M. Kennedy Community Health Center (and other primary care providers). (and other primary care providers). This TA helped support a data collection system that tracks whether referral occurred and was completed and whether uninsured patients were linked to coverage.
